✅ What You'll Do as a Home Health LPN
- Provide direct nursing care to patients in their homes across assigned territories
- Administer medications, wound care, IV therapy, and other treatments
- Educate patients and caregivers on disease processes, medications, and home care techniques
- Collaborate closely with RN Case Managers and the full interdisciplinary care team
- Ensure accurate and timely clinical documentation

What You'll Need to Join as a Home Health LPN:
- Active LPN license in the state of Massachusetts
- At least 2 years of nursing experience (1 year of home health preferred)
- Reliable transportation and a valid driver's license
- Compassion, attention to detail, and commitment to patient-centered care
