Job DetailsJob Location: Baker Building - Astoria, OR 97103Position Type: Full TimeSalary Range: $26.78 - $34.95 HourlyThis position qualifies for a hire on bonus of $2000 with 1/2 paid with 1st check after hire and 1/2 paid with 1st check following 6-month probationary period. Clatsop Behavioral Healthcare (CBH) has moved most positions to a 4 day 9 hour work week with paid 30 minute lunches to promote a better work life balance. Wages have also been increased for our valued current and future employees. Clatsop Behavioral Healthcare has an immediate opening for a full time Community Support Services (CSS) Case Manager in our Open Door program. The primary purpose of this position is to provide services for adults diagnosed with severe mental health issues that may make it difficult to accomplish daily tasks. This position works as part of a collaborative process to assess, plan, implement, coordinate, monitor, and evaluate the options and services required to meet an individual's health and human service needs.  Provide treatment services which provide individual case management, medication assistance, skill building, groups, and a variety of socialization activities. Open Door case management includes service planning and coordination, family and social relationship building, referral to substance abuse treatment and support, housing assistance, referral to supported employment and education, wellness self-management, problem solving, skills development, (including basic personal care and safety skills, grocery shopping and cooking, purchasing and caring for clothing, household chores, using transportation, assessing other community resources), health education and support, medication access, financial management, and empowerment and self-help. Support and assist clients with OHP enrollment and coordinating maintained access to OHP benefits. Essential Functions/Major Responsibilities: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Interview clients to obtain information concerning medical and psychosocial history or other pertinent information. Observe clients to detect indications of abnormal physical or mental behavior of clients. Plan with clients and provide services such as behavior modifications, skills training, education on symptoms and symptom management, and stress reduction treatment, to assist clients in understanding and controlling symptoms and other interpersonal problems. Provide sensitive community outreach to clients and natural support providers of clients by visiting them in their homes, workplaces, or in the community. Provide supervision and guide activities of clients at Open Door setting. Change method and degree of treatment when indicated by clients' reactions as outlined in the Recovery (Treatment) Plan. Discuss progress toward goals with clients, clients' families, and other natural support providers. Consult with supervisor, team lead, medical director, nurse practitioner, primary care Physician and other specialists concerning treatment plans and makes recommendations to the overseeing Qualified Mental Health Professional (QMHP) regarding updates to Recovery (Treatment) Plans. Conduct relaxation exercises, facilitate skills training groups, teach life skills, and other evidence-based services. Refer clients to supportive services to supplement treatment and counseling. Participate in protective service investigations as directed by supervisor. Exercise a high degree of understanding and compliance with accepted standards of client care. Understand and support the mission of the agency and adhere to all agency policies and protocols. Obtain Releases of Information to be able to consult and coordinate care with other community agencies. Participate in regular team and agency meetings. Maintain productivity in keeping with agency expectations and standards. Coordinate discharge planning with all treatment providers. Remain flexible to the changing demands of a mental health system and the needs of the agency. All CBH Staff are Mandatory Abuse Reporters, per the State of Oregon, and as such all staff must be aware of and follow Mandatory Abuse Reporting Policies. Ensure client confidentiality is maintained and adheres to all ethical and professional standards. Complete and maintains all clinical documents and client files according to compliance standards with the State of Oregon Administrative Rules, Care Oregon and Quality Assurance standards established at Clatsop Behavioral Health. This includes, but is not limited to, the timely completion of Progress Notes (Service Tickets). Maintain clients' charts in keeping with Oregon Administrative Rules and agency quality assurance standards, protocols, and procedures and timelines. Other duties as assigned. Education/Experience Requirements: Bachelor's Degree in Social Work, Behavioral Health, Psychology (Trauma informed care) or Counseling or an acceptable combination of education and experience. Ability to obtain Qualified Mental Health Associate (QMHA) certification through MHACBO.  2-4 years' prior experience in the field of case management working with families and children in the mental health/child psychology arena, developmental disabilities or drug and alcohol issues. Knowledge of Diagnostic and Statistical Manual of Mental Disorders (DSM)-5, Social Workers Code of Ethics, and Quality Assurance Standards of Clatsop Behavioral Healthcare, American Society of Addiction Medicine (ASAM), and the State of Oregon Administrate Rules applicable to this position. Knowledgeable about civil commitment process to explain to clients/family members. Prior experience working with the therapeutic process and modalities, dependency courts and Department of Human Services including their policies, procedures and practices. Experience with electronic health recordkeeping and protocols for assessing health information. Experience working with community agencies such as Probation, Department of Human Services, Court Appointed Special Advocates and School to collaborate on client services. Working knowledge of developmental disabilities and mental health issues facing clients. Knowledge of Trauma -Informed therapy. Knowledge of human behavior of individuals diagnosed with a serious mental illness and addiction issues. Skilled at working as a member of an interdisciplinary team and completing required documentation. Articulate well, speak precisely, and think and write clearly. Knowledge or ability to learn about Medicaid, Oregon Health Authority (OHA), and related State and Federal health care regulations to include Oregon Administrative Rules (OARs) and statutes.  Excellent oral and written communication skills.  Strong attention to detail, highly organized. Knowledge of the Health Insurance Portability and Accountability Act (HIPAA) and the importance of maintaining confidentiality. Ability to pass a criminal background check. Have and maintain an Oregon or Washington Driver’s license and meet the agency’s insurance standards.      Preferred: Skilled at remaining calm in stressful situations and de-escalating potential aggression from clients. Skilled at leading and facilitating groups. Skilled at conducting training in groups. Knowledge of Clatsop Behavioral Health billing process, the Electronic Health Records, and basic intake process. Ability to learn and adopt the policies and procedures of the agency.
