As part of daily activities, pharmacists routinely assess pertinent patient information, such as demographic data, vital signs, laboratory values, medication regimens, medication compliance, and health insurance coverage. Pharmacists collaborate with all members of the healthcare team, patients, and their caregivers to provide value-based, patient centered care.
