Responsibilities:
- Conduct comprehensive assessments to evaluate clients' mental health needs.
- Develop and implement individualized treatment plans based on assessment results.
- Collaborate with multidisciplinary teams to ensure holistic care for clients.
- Provide ongoing support and counseling to clients and their families.
- Maintain accurate and thorough documentation of client progress and treatment plans.
- Stay updated on the latest best practices and evidence-based interventions.
- Participate in training and development opportunities to enhance professional skills.
- Adhere to ethical guidelines and maintain confidentiality in all client interactions.
Requirements:
- Must be willing to see clients in COMMUNITY-BASED SETTINGS (CORE Service Based)
- Must possess the clinical skill, professionalism, and ethical judgment necessary to effectively serve vulnerable populations diagnosed with severe and persistent mental illness (SPMI) and complex behavioral health needs.
- Master's degree in Counseling, Social Work, or a related field.
- Current licensure as LPC, LCSW, or LMFT in the state of Georgia.
- Minimum of 2 years of experience in behavioral or mental health assessment.
- Strong interpersonal and communication skills to build rapport with clients.
- Ability to work collaboratively in a team-oriented environment.
- Knowledge of evidence-based therapeutic practices and interventions.
- Compassionate demeanor with a commitment to client-centered care.
- Proficient in using electronic health record systems for documentation.
- Candidates must have reliable internet service, access to a working computer/laptop, and a functioning cell phone to support communication, documentation, and remote job-related tasks.
