The Diabetes Education Nurse monitors program metrics, tracks clinical and behavioral outcomes, and contributes to quality and process improvement initiatives, including grant funded projects. This position presents educational content clearly and effectively, with sensitivity to the psychosocial needs surrounding pregnancy, birth, postpartum, and parenting.
Education:
- Graduate of an accredited school of nursing
- Bachelor Science Nursing (BSN) required
- Certified diabetes educator: credentialed as a diabetes education specialist or diabetes clinical management specialist preferred
Licenses/Certification:
- Current RN Massachusetts Licensure
- Active Basic Life Support (BLS) certification through American Heart Association (AHA) or American Red Cross
Required Qualifications and Skills:
- General interest and knowledge of prenatal/postnatal nutrition and diabetes self-management as related to perinatal care (prenatal and post-partum)
- Experience providing patient education and counseling to support chronic disease management.
- Demonstrated ability with good organizational, analytical and interpersonal skills
- Strong ability to communicate complex medical information in a clear, patient-centered manner that supports health literacy.
- Demonstrated experience in clinical teaching and documentation of patient management
- Computer skills include Clinical Documentation, Outlook, Microsoft Office, and presentation applications.
Preferred Qualifications and Skills:
- Certification as a Gestational Diabetes Educator is preferred but not required; candidate should demonstrate the ability and willingness to learn gestational diabetes management.
- Demonstrated interest in staying informed on current evidence-based practices, emerging clinical research, available patient resources, and opportunities to support program and process improvement.
- Demonstrated ability to provide culturally responsive care and adapt educational materials for diverse patient populations.
- Experience collaborating within a multidisciplinary care team, including physicians, dietitians, and care navigators.
- Strong presentation and facilitation skills for both individual and group patient education.
Essential Job Functions:
- Provide comprehensive diabetes self-management education to patients in the OB/GYN outpatient setting, including individualized and group education sessions.
- Educate patients on diabetes management during pregnancy, postpartum, and reproductive health, including glucose monitoring, medication adherence, nutrition, and lifestyle modifications.
- Develop, implement, and regularly update the OB/GYN-specific diabetes education curriculum, ensuring alignment with current evidence-based guidelines and best practices.
- Collaborate with obstetric providers, nursing staff, maternal care navigators, and registered dietitians to coordinate patient care and support comprehensive diabetes management.
- Promote patient engagement and improve health literacy by providing clear, accessible education and resources that empower patients to actively participate in managing their diabetes during pregnancy and postpartum care.
- Document all patient education encounters, assessments, and care plans accurately in the electronic health record (EHR) in accordance with organizational and regulatory standards.
- Participate in quality improvement and process improvement initiatives (e.g. grant-funded programs)
- Maintain knowledge of current clinical guidelines, diabetes management practices, and maternal health standards through ongoing professional development.
