The Emergency Solutions Case Facilitator operates as a team member under the supervision of the Emergency Solutions Team Manager.
Primary job duties include 24/7/365 on-call for crisis intervention services and acting as community liaison between Northeastern Center and referral sources. On call services include telephone, face-to-face, and telehealth interactions with clients. Â Follow-up action on previous emergency calls and maintaining positive working relationships with referral sources (hospitals, nursing homes, etc.) are other essential tasks.
A Bachelors Degree related to mental Health and lived experience is required.Â
Lived experience is defined as either a personal experience in their own recovery form mental health or substance use or direct experience supporting a loved one in their recovery journey.Â
Certified Peer Support Professional Certification (Must be obtained within 90 days of hire).Â
Must possess a valid state issued drivers license.
Second shift, Full time. 2p-10p M-F, alternating on-call.Â
