The nurse/case manager role on the Assertive Community Treatment (ACT) teams is primarily
focused on nursing services to adults diagnosed with serious, long-term mental illness. Services
shall include needed nursing and medical care as well as case management coordination and
activities of daily living assistance to assure a comprehensive and holistic approach to meet
individual needs and assist the consumer in obtaining and maintaining psychiatric stability.
Education: RN; with Bachelor's degree in nursing or related health/mental health field preferred;Â
Licensed Practice Nurse accepted.
Experience: Minimum 2 years nursing experience; preferably in working with adults with long-term serious mental illness.
Licenses: As required by degree.
First shift,full time Monday - Friday, 8a-5p
Salary based on experience.
