A Case Facilitator I - Child and Adolescent operates under supervision of the assigned Area Director or designated QBHP/LP. A member of a multidisciplinary treatment team, primary job duties include providing case management and activities of daily living skills to child and adolescent consumers.
o   Links, monitors, and refers clients to appropriate community resources
o   Coordinates care with appropriate internal and external parties
o   Ensures appropriate behavioral health services are provided
Education: High School diploma or equivalent.
Experience: Must meet OBHP requirements.=Two years’ experience in mental health or community services preferred.
Licenses/ Certification: Must provide a valid state issued drivers license.
Shift: first shift, full time, M-F, No holidays or weekends.
Salary based on experience.
