The Discharge Planner opportunity is a key member of the Lighthouse Clinical Services Team who will integrate and coordinate a patient centric therapeutic strategy with a keen focus on patient care; ensuring that delivery of high-quality and cost-effective strategic planning is consistent with the mission, vision, and values of Universal Health Services and in accordance with government regulation, licensing and accreditation requirements.  The Discharge Planner is responsible for comprehensive assessment of persons whose lives are disrupted or complicated by mental illness, behavioral disturbance, substance issues or inability to function or maintain in the community. The Discharge Planner is responsible for

  • coordinating the discharge plan offered to the patient
  • interfacing with the treatment team/managed care organizations to ensure quality patient care,
  • collaborating with next level professionals for continuity of care,
  • obtaining certification for services for next level of care/ providers. 
  • functioning as a member of the multidisciplinary team and participate in the community relations program of the organization
  • participating in continuous performance improvement activities
  • communicating with outpatient referral sources, families/guardians, and outpatient aftercare providers.

  • Masters Degree in a behavioral science related field (social work, clinical mental health, clinical counseling, marriage and family therapy)
