What You’ll Do (High Level)
- Evaluate patients and create individualized plans of care
- Deliver hands-on treatment and progress patients toward meaningful goals
- Partner with a supportive, multidisciplinary team (case managers, respiratory therapy, authorizations)
- Keep documentation clear and timely so care stays coordinated and uninterrupted
- Help create a calm, encouraging environment where patients feel safe to try, rebuild, and improve

What You Need
- Graduate of a CAPTE/APTA-accredited PT program
- Active PT license (or eligibility) in the state of hire
- BLS certification (or willingness to obtain)
- A patient-first mindset and strong communication
