Job DetailsLevel: ExperiencedJob Location: Sherman 620 - Aurora, IL 60505Position Type: Full-TimeEducation Level: Bachelors DegreeSalary Range: $24.50 - $24.50 HourlyTravel Percentage: RegularlyJob Shift: DayJob Category: Health CareThe Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges, those who have suffered a trauma, and those at risk to enjoy lives of dignity and purpose.  We are looking for a Case Manager Mental Health Professional or Qualified Mental Health Professional (MHP/QMHP) in Aurora, IL who exemplifies that mission, and wants to work for an organization that makes a difference.” Schedule: Monday - Friday 8:00AM - 4:30PM OR Tuesday - Friday 8:00AM - 6:30PM What will you be doing?  CM MHP/QMHP is responsible for developing and implementing individual programs to maximize the clients’ independent community living skills.  The CM MHP/QMHP must complete documentation of mental health services provided in accordance with state and agency standards as applicable.    The CM MHP/QMHP is expected to work with a team of behavioral health service providers within and outside the agency to ensure positive client-centered approach.   Essential Job Responsibilities Work scheduled hours and be flexible to meet client and program needs  Responsible to provide program coverage when scheduled  Provide effective services for clients’ individual needs and provide direct implementation of treatment goals for assigned caseload  Meet with all assigned clients on regular basis depending on program and client needs; turn in daily activity logs  Assure the coordination of services in the community to meet client needs (i.e. Financial, legal, advocacy, medical, etc.)  Assist clients with activities of daily living, including apartment maintenance, safety of living arrangement, and provide client training consistent with the IM+CANS  Facilitate communications and coordination of services with other AID staff and professionals in the community involved in client care  Assist clients in paying bills, budgeting and banking per program procedure  Oversight or management of client funds when necessary (dual signature accounts, AID rep. Payees)  Obtain and maintain client benefits (Social Security, Medicaid, Link Card, Etc.)  Complete authorizations, reauthorizations and spend-down paperwork in a timely fashion  Complete and sign all MRO documentation of services provided within 48 hours using Cx360  Meet or exceed monthly minimum number of service hours productivity standard.  Develop, review, and revise the IM+CANS and complete corrections within the timeframe allotted   Complete all required case management documentation and client paperwork (assessments, IM+CANS, consents, agency paperwork) on a timely basis  Maintained and update client records in Cx360 per agency procedures including scanning necessary paperwork into Cx360 per procedure  Provide guidance to MHP and RSA staff regarding implementation of IM+CANS goals/objectives  Maintain professionalism and good boundaries when working with clients, coworkers, and outside agencies  Assure compliance with all agency, state and federal regulations while providing services and completing assignments.  Review and follow updated policies and procedures  Obtain Input from clients, families, guardians and other staff on how to improve residential services    Acquire and maintain required certification and mandated trainings as well as any other trainings assigned by Manager  Complete and submit QI corrections within time frame allotted  Attend monthly team meetings and clinical supervisions and be a positive contributor  Recognize emergency situations and take appropriate action.  Contact Manager and Director per procedure.  Complete necessary paperwork correctly (incident reports, petitions, encounters/non-billable notes).    What will we provide Full Time employees. What will you bring to the table?  Education:     Bachelor’s degree in Human Services or related field required for MHP; Masters Degree in Human Services for QMHP.  Experience:     Experience working in social services required.  Physical:     Navigation of stairs No lifting restrictions. Ability to provide services in clients’ homes.
