Location: Austin, TX

Facility: Childrens Comprehensive Care

Department/Specialty: Childrens Comprehensive Care

Schedule: Full time | Monday-Friday 8am-5pm

How you'll make an impact in this role

  • Guide seamless care transitions by serving as the primary liaison between patients, physicians, leadership, and community partners to ensure aligned, compassionate patient care.

  • Elevate standards of care through active coordination of clinical treatment plans, quality improvement initiatives, and educational programming across the department.

  • Drive process improvements by tracking key patient outcomes and analyzing performance data to optimize operational efficiency and healthcare delivery.

  • Bridge healthcare access gaps by connecting uninsured and underinsured patients with vital community resources and follow-up care pathways.

What minimum qualifications you'll need

Licensure / Certification / Registration:

  • Registered Nurse credentialed from the Texas Board of Nursing required.
  • BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

What additional requirements you'll need

  • Core Qualifications & Technical Skills: Extensive background in case management, strong computer proficiency, and the adaptability to navigate flexible work requirements.

  • Field & Client Requirements: Hands-on experience conducting community and home visits (reliable personal transportation required), with a preference for bilingual candidates and those with pediatric care experience.
