Job DetailsJob Location: Spicewood - Austin, TX 78759Position Type: PRNJob Shift: Flexible JOB TITLE:        RN Admissions                                         DEPARTMENT:  Access & Marketing   POSITION SUMMARY Identify, assess and admit appropriate patients for hospice services.   WORKING RELATIONSHIPS REPORTS TO:   Team Leader SUPERVISES:   None   INTERPERSONAL RELATIONSHIPS: All comments, actions and behaviors have a direct effect on the public’s perception of Hospice Austin. Interactions with patients, family members, physicians, referral sources, visitors, co-workers, etc. must be in a manner that is courteous, respectful, cooperative and professional. This behavior should promote an atmosphere of teamwork and positive relations.   ESSENTIAL FUNCTIONS Performs admissions process. (80%) Accepts patient referral; visits patient in residential, nursing home or hospital setting. Performs assessments, i.e. vital signs, pain assessment, breath sounds, etc. Determines appropriateness for admissions. Completes documentation for admission to services if appropriate. Consults with supervisors, MDs, etc. regarding appropriateness for hospice services. Refers patient back to referral source for additional follow-up as needed. Teaches patient and caregivers (including nursing home staff) on end of life care – e.g. disease process, death and dying, comfort measures, medication use, side effects, etc. Initiates Plan of Care. Provides psychosocial support to patients and families. Coordinates patient care. (20%) Initiates coordination of personal care, DME, and obtaining medications and medical supplies. Documents in a timely manner patient assessments, patient contacts, physician orders and progress notes. Acts as a liaison between patient and physicians, other care team members and referral sources. Other duties as assigned. Qualifications Graduation from an approved school of professional nursing as determined by the Board of Nurse Examiners required. Licensed as a Registered Nurse by the Texas Board of Nurse Examiners required. Valid Texas driver’s license, auto liability insurance and safe driving record required. CPR certification from a recognized entity following AHA and OSHA standards. 1-year previous hospice experience.
