We're seeking a licensed Physical Therapist Assistant (PTA) to join our growing rehabilitation team in a Skilled Nursing Facility (SNF) in Avon, Ohio. This role offers the opportunity to work with older adults in a collaborative post-acute care environment focused on improving mobility, independence, safety, and quality of life.

Requirements

 What You'll Do

  • Provide physical therapy treatments in accordance with individualized care plans
  • Help patients improve mobility, strength, balance, endurance, and functional independence
  • Communicate patient progress and recommendations to the supervising Physical Therapist and interdisciplinary care team
  • Collaborate with nursing staff, physicians, families, and caregivers
  • Participate in care planning and discharge planning activities
  • Educate patients, families, and facility staff on therapy programs and safety strategies
  • Maintain timely and accurate electronic documentation and billing records
  • Assist with restorative and functional maintenance programs
  • Recommend equipment and treatment interventions that support successful patient outcomes
  • Support a positive rehabilitation environment focused on quality care and service excellence

Qualifications Required

  • Graduate of an accredited Physical Therapist Assistant program
  • Current PTA license in the State of Ohio
  • Strong communication and interpersonal skills
  • Commitment to patient-centered care
  • Ability to work collaboratively within an interdisciplinary healthcare team

Preferred

  • Experience in Skilled Nursing, Rehabilitation, Long-Term Care, Post-Acute Care, or Geriatrics
  • New graduates are encouraged to apply
