The successful candidate:
***Shift- M-F 8:00AM-4:30PM. May occasionally include evenings, nights, or weekends when coverage needed. Experience in internal medicine, critical care, hematology/oncology highly preferred.***
In this role, the pharmacist works as part of the multidisciplinary health care team to directly care for patients. They will also provide central staffing coverage, as required.
Coordinates all clinical pharmacy functions for the acute care areas as assigned to help achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams.
Completes patient profile reviews and participates in daily clinical care rounds, huddles, and meetings as an integrated member of a multidisciplinary team
Reviews medication regimens for appropriateness, contraindications, and potential interactions.
Helps monitor therapeutic plans and medication treatment.
Orders, reviews, and acts upon appropriate laboratory tests and other diagnostic studies necessary to support a patient's medication therapy regimen.
Performs pharmacokinetic monitoring, renal dose adjustments, anticoagulant management, glycemic management, pain management, prophylaxis review, IV to PO conversions, and antimicrobialstewardship as necessary.
JOB QUALIFICATIONS:
Education: Graduate of ACPE-accredited Pharmacy School or School with ACPE accreditation pending
Licensure: License in good standing with Arizona State Board of Pharmacy
****PREFERRED JOB QUALIFICATIONS****:
Experience: internal medicine, critical care (2+ years inpatient hospital experience or completion of PGY1)
Desired experience: hematology/oncology
