This employee assumes responsibility and accountability for assigned patients (adult and geriatric).
  • Works with supervision within policy and program guidelines
  • Provides professional nursing care, incorporating the nursing process, the
    environment, technical equipment, and other health care members
  • Provides input for program maintenance and development
  • Works independently with limited direct supervision using the company policy and professional judgment
JOB FUNCTIONS
  • Oversees the direction, coordination, and evaluation within the cardiovascular rehabilitation program
  • Administers nursing care to cardiopulmonary patients
    • includes assessment, planning, implementation, and evaluation of the patient's plan of care in coordination with the physician
  • Demonstrates awareness of legal issues in all aspects of patient care and unit function
    • strives to manage situations in a reduced risk manner and promote patient safety
  • Solves issues with staff, physicians, patients, families, and ancillary departments
  • Assumes job responsibilities of other team members during their absence
  • Assesses the cardiovascular inpatient population to provide educational plans
  • Identifies individual risk factors associated with cardiac and pulmonary disease, and assists with risk stratification
  • Identifies the need for reevaluation of each patient if the patient's condition changes and warrants intervention
  • Supervises patients during exercise sessions, monitoring vital signs, including heart rate, blood pressure, and oxygen saturation levels to ensure safety during exercise sessions
  • Provides patient/family education and counseling for identified risk factors and develops life-style modifications pertaining to cardiopulmonary disease and related health issues
  • Maintains accurate and detailed records of the patients progress, treatments, and care plans in compliance with departmental and hospital policies for medical records
  • Develops individualized educational plans of care based on patient's goals and expectations
  • Evaluates, updates, and revises plan of care to facilitate achievement of planned and expected outcomes
  • Initiates emergency procedures according to policy
  • Reviews patient history and other medical record to develop a plan of care that will include behavior modification through activity and diet intervention
  • Facilitates phase I to phase II transition process for cardiac rehab
    • actively assigns patients to phase II who qualify for the program
    • Provides follow-up phone calls as necessary
  • Actively participates in the patient education and community based programs by organizing classes, communicating with speakers and participants, as well as providing actual classroom instruction
  • Demonstrates competency in assessing emergent patient situations that require physician or emergency medical interventions
Qualifications
Education
  • Graduate from an accredited school of professional nursing, Required
  • BSN, Preferred
Work Experience
  • One year in a hospital-based cardiology related patient care unit, Required
  • Critical care, Required
  • monitoring and interpretation of electrocardiograms, Required
Knowledge, Skills, and Abilities
  • Extensive understanding of cardiac disease, anatomy and physiology, risk stratification, exercise physiology, and diet and nutrition are essential
  • Intermediate computer
    • Microsoft Word, Excel, Outlook, and Windows
  • use the information systems pertinent to the job
Licenses and Certifications
  • Advanced Life Support (ALS) certification from the American Red Cross or American
    Heart Association certification, Required
  • Licensed as a Registered Nurse in the state the services are provided, Required
