RESPONSIBILITIES:
Evaluates sick, urgent care, transitional care and primary care patients Performs annual physical exams as needed Educates patients on maintaining proper health Evaluates, makes recommendations, co-manages and treats patients' medical needs Participates in an interdisciplinary team providing care and coordination of our patients, ensuring the highest level of care
