Mercy Medical Center is seeking dedicated Clinical Pharmacists to join our acute care and transition teams! Mercy pharmacists are valued members of the health care teams. Pharmacists work in patient care areas and rotate through the central inpatient pharmacy. Pharmacists are involved in all aspects of patient care including, but not limited to, bedside rounds with the provider and nurse, medication dosing/monitoring, antibiotic de-escalation and precepting students and residents. When working in the central pharmacy, the pharmacists have responsibility for aspects of pharmaceutical care, oversight of sterile compounding and ensuring accurate medication distribution. Job Duties
- Reviews, evaluates and interprets medication orders for appropriateness.
- Oversees accurate preparation and distribution of medication orders.
- Collaborates with physicians, nurses and other members of the healthcare team to ensure appropriate medication therapy.
- Documents clinical interventions.
- Detects, monitors, and documents adverse drug reactions and medication errors.
- Provides clinical pharmacy services through consultation, drug information, teaching and/or committee work.
- Participates in the drug therapy management of medical emergencies, as needed
- Demonstrates evidence of continued professional development.
- Participates in the training and education or new employees and pharmacy students.
- Develops and implements quality improvements to ensure safe and cost-effective management of pharmaceuticals.
- Assists with special projects pertaining to specific area of interest and/or expertise.
- Maintains a working knowledge of and complies with pharmacy policy and standards of work and the laws, rules, and regulations that govern the distribution of medications.
- Responsible for intra-departmental communication.
- Promotes effective working relations and works effectively as part of a team inter- and intra-departmentally to facilitate the department's ability to meet its goals and objectives.
- Follows Mercy's safety guidelines, carries out job-specific safety duties and responsibilities, and promptly reports any unsafe conditions, situations, incidents and injuries.
Knowledge, Skills and Abilities
- Ability to meet the pharmaceutical care needs of the following patient age groups: neonatal, pediatric, adolescent, adult, and geriatric.
Professional Experience
- Two years as a licensed pharmacist with enhanced clinical responsibilities preferred.
- Successful completion of an Accredited American Society of Health-System Pharmacist (ASHP) Residency Program preferred.
Education
- Doctor of Pharmacy (PharmD) or Bachelor of Science (BS) degree in pharmacy, required.
Licensure, Certification, Registration
- Must have current Iowa Pharmacist License or be eligible for Iowa Board of Pharmacy pharmacist licensure. Employment is contingent upon being licensed by the Iowa Board of Pharmacy by start date.
- Current certification in Basic Life Support (BLS) and Advanced Cardiac Life Support, in accordance with the American Heart Association preferred.
- Board Certified Pharmacotherapy Specialist (BCPS) preferred.
- Iowa Dependent Adult Abuse & Iowa Child Abuse Mandatory Reporter training required. Employee must submit certification of completed training to Human Resources prior to start or transfer date.
