This position supports Mercy's philosophy of patient centered care by performing a variety of clinical services under the supervision of a registered pharmacist. In collaboration with the registered pharmacist, the Pharmacist Intern – Clinical will work as part of the patient’s care team. Responsibilities may include: working in the Mercy Anticoagulation Clinic; providing telephonic follow-up of resulted lab values; providing medication dosing plans; or medication education directly to patients. Additional clinical responsibilities may be assigned based upon Mercy’s patient care needs.Job Description
Job Duties
- Maintains a working knowledge of pharmacy terminology, pharmacology, and pharmacotherapy.
- Appropriately triages phone calls and takes detailed phone messages. Responsible for intra-departmental communication.
- Responsible for intra-departmental communication.
- Under the Supervision of a Clinical Pharmacist, collaborates with physicians, nurses and other members of the healthcare team to ensure appropriate medication therapy, documents clinical interventions, and provides clinical pharmacy services through consultation, drug information, teaching, committee work, and/or special projects pertaining to areas of interest and/or clinical expertise.
- Maintains a working knowledge of and complies with pharmacy policy and standards of work and the laws, rules, and regulations.
- Promotes effective working relations and works effectively as part of a team inter- and intra-departmentally to facilitate the department's ability to meet its goals and objectives.Â
- Conducts telephonic follow-up of resulted lab values, and provides medication dosing plans and education directly to patients.
- Places standing orders as allowed by protocol.
- Interacts directly with pharmacists, pharmacy technicians, patients and caregivers as part of the patient’s care team.
- Enters documentation in the patient's chart as needed.
- Follows Mercy's safety guidelines, carries out job-specific safety duties and responsibilities, and promptly reports any unsafe conditions, situations, incidents and injuries.
Knowledge, Skills and Abilities
- Ability to operate basic computer applications.
- Ability to calculate dilution and dosing programs in the clinical setting.
Professional Experience
- One year of pharmacy technician experience, preferred.
Education
- Completion of at least the first professional year of pharmacy school in an accredited College of Pharmacy required.
Licensure, Certification, Registration
- Registration with the Iowa Board of Pharmacy as a certified pharmacy technician or pharmacy technician trainee until the intern is eligible to register with the Iowa Board of Pharmacy as a Pharmacy Intern.
- Registration as an Iowa Pharmacist Intern shall be obtained by August 31st but no later than September 15th of the first year of pharmacy school.
- Iowa Dependent Adult Abuse/Iowa Child Abuse Mandatory Reporter training required. Employee must submit certification of completed training to Human Resources prior to start or transfer date.
