What You’ll Do: Assesses and identify the client’s holistic health care needs/concerns as part of a family and community.  Assesses social/emotional factors that are related to the patient’s illness, need for care, response to treatment, and adjustment to care. Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols. Implement patient care plan activities based on treatment goals towards independent living for the patient/family. Serve as resource for staff education & training, participate in case conferences as needed. Support Hospice, and Home Health What You’ll Bring: Degree from a school of social work accredited by the council on social work education.  Maine Board of Social Work Licensure:  at least LSX Two years of Medical Social Work preferred Current CPR card required.  A valid Drivers License and reliable car with insurance.
