This role supports individuals with developmental disabilities and their families, coordinates person-centered services, and ensures care aligns with each person’s unique goals. You’ll supervise direct support staff, provide hands-on support when needed, and manage essential administrative tasks—all while collaborating with community partners to create meaningful, positive outcomes. Summary of Duties: Coordinate person-centered services for individuals with developmental and intellectual disabilities and autism in home and community settings Ensure services and care plans align with each person’s unique needs and goals Supervise direct support staff and provide guidance on service delivery Deliver some direct care as needed Manage essential administrative tasks related to service coordination Collaborate with team members and community partners to support positive outcomes Apply creative problem-solving to address challenges in service delivery Compensation and Benefits WCMHS provides a comprehensive benefits package including health, dental, vision, retirement, and a generous paid time off package including flexible holidays and 12 vacation days in year one. The pay range for this position is $26.59-$27.67 per hour. QualificationsSummary of Qualifications High school completion or equivalent; BA preferred Three or more years’ experience providing services to individuals with developmental and intellectual disabilities and autism Knowledge of Behavioral Support Plans, Communication Plans, and Individual Support Agreements for individuals with intellectual and developmental disabilities and autism Ability to collaborate and work productively with internal and community partners Ability to document work/services provided into an electronic records system with a high level of accuracy and timely completion Team leadership experience strongly preferred Valid driver’s license, an acceptable driving record, and consistent, regular access to a safe, reliable, insured vehicle required Ability to provide services and complete treatment notes in English Working Conditions Working in a variety of locations including program spaces, residential environments, and in the community, working with clients who may have difficult behaviors, transporting clients with one’s car is required, and the ability to lift up to 50lbs.
