Care Coordinator - Community Health Worker

Grace Health is seeking a dedicated and compassionate Care Coordinator to join our interdisciplinary clinical care team. This role focuses on promoting preventive healthcare, increasing access to services, and addressing barriers to care for our patients.

The Care Coordinator works directly with patients to support chronic disease management, routine primary care visits, and overall health improvement. This position plays a key role in coordinating communication between patients, providers, and community resources to ensure continuity of care.

Licenses & Certifications: Community health worker training and/or certification.
