The SHARE Clinician plays a critical role in ensuring the delivery of high-quality, recovery-oriented services. Working directly with individuals in community settings, the Clinician completes comprehensive assessments, evaluates behavioral health needs, and develops individualized treatment and rehabilitation plans, including the Individualized Rehabilitation Plan (IRP). The Clinician provides therapeutic support, crisis intervention, clinical consultation, and ongoing monitoring of participants' progress toward recovery goals.
In addition, the Clinician serves as the clinical leader for the SHARE team by providing guidance, oversight, and supervision to Case Managers. This includes supporting staff in identifying appropriate interventions, ensuring clinical best practices are followed, promoting compliance with documentation and regulatory requirements, and enhancing the overall quality of care provided to consumers. Through collaboration with internal and external providers, the Clinician helps ensure individuals receive coordinated, integrated services that promote housing retention, recovery, independence, and an improved quality of life.

Responsibilities:
- Perform therapeutic rehabilitative skill development
- Maintain accurate documentation and client records
- Recommend referral services as needed
- Develop, implement and maintain client IRP
- Provide education to client systems related to nutrition, symptom management, medication safety and management, and discharge planning
- Provide transportation to clients as needed
- Provide crisis intervention services as needed
- Graduation from an accredited college or university in Social Work, rehabilitation counseling, psychology, criminal justice, counseling, or other related behavioral healthcare or counseling
- LAC/LSW required
- One year of experience in the psychiatric/mental health field
- Valid N.J. drivers license with less than 6 points
