About the Role:

Responsibilities:
- Conduct comprehensive assessments of clients' health and social needs.
- Develop and implement individualized care plans in collaboration with healthcare teams.
- Monitor client progress and adjust care plans as necessary.
- Provide education and resources to clients and their families.
- Facilitate communication between clients, families, and healthcare providers.
- Document all interactions and maintain accurate client records.
- Advocate for clients to ensure they receive appropriate services and support.
- Stay updated on best practices and regulations in case management and nursing.
Requirements:
- Registered Nurse (RN) license in the state of Indiana.
- Minimum of 2 years of nursing experience, preferably in case management.
- Strong assessment, communication, and organizational skills.
- Ability to work independently and as part of a team.
- Experience with electronic health records (EHR) systems.
- Compassionate demeanor with a client-centered approach.
- Certification in case management (CCM) is a plus.
- Willingness to engage in ongoing professional development.
