About the Role:
Trust Home Care Inc is looking for a dedicated and compassionate Home Health Physical Therapy Assistant to join our growing team in Harford County. In this rewarding role, you'll make a meaningful difference by delivering hands-on rehabilitative care directly to patients in the comfort of their own homes. Responsibilities:
- Implement physical therapy treatment plans under the supervision of a licensed Physical Therapist
- Provide in-home therapeutic exercises, gait training, and functional mobility assistance
- Monitor and document patient progress, reporting changes in condition to the supervising PT
- Educate patients and family members on home exercise programs and safety techniques
- Assist patients with activities of daily living (ADLs) to promote independence and recovery
- Maintain accurate and timely clinical documentation in compliance with home health regulations
- Coordinate care with interdisciplinary home health team members to ensure continuity of care
Requirements:
- Active Maryland PTA license in good standing
- Graduate of an accredited Physical Therapist Assistant program
- Previous home health or outpatient PT experience preferred
- Valid driver's license and reliable transportation for home visits in the Bel Air, MD area
- Strong knowledge of rehabilitative techniques, therapeutic exercise, and patient safety
- Excellent communication, time management, and organizational skills
- Compassionate, patient-centered approach with a commitment to quality care
