This position will provide Medication -Assisted Treatment (MAT) services to a percentage of the patient population served in our Primary Care Health Center. Community Health & Wellness Partners is an integrated primary care community health center that utilizes a team-based care model, sharing patient-care responsibilities among members of a team. In addition to providing relevant direct and appropriate clinical care, treatment and diagnosis to patients within the scope of practice for a primary care nurse practitioner, the nurse practitioner works in tandem with the team’s physician to function as a Team Leader in overseeing and directing the full operations of a team-care model, providing whole-person, quality care.  Function as a fully accountable colleague with physicians and clinics through collaborative care agreements, and strives to support CHWP’s quality initiatives and requirements necessary and relevant to a Federally Qualified Health Center, and various certification programs, i.e. HRSA, PCMH, Joint Commission, etc.  Primary Responsibilities Achieve Results Effect improvements in patient well-being through accurate and timely diagnosis and delivery of relevant, high quality medical care. Ensure the delivery of competent, accurate, medical care and treatment to all patients as assigned. Meet or exceed all measurable standards of care, including overall clinical, quality and productivity goals. Ensure accuracy, efficiency, and appropriate confidentiality with patient charting and related recordkeeping and administrative functions as assigned  Operational Excellence Ensure the delivery of all medical treatment and care is consistent with all regulatory, accreditation, and professional standards, including clinical policies and quality initiatives. Understand and ensure all practices and procedures related to the delivery of medical services are consistent with contracted protocols and procedures and quality initiatives. Serves as a Team Leader to co-manage and support the full operations of the team-care model Relationship Management Develop and ensure favorable relationships with all patients and their families. Achieve commitments from patients to adhere to an effective plan of care, and ensure all patient services are delivered in a caring and professional manner. Develop and ensure ongoing, positive relationships and collaboration with other members of the Team and CHWP staff. Ensure the success of each clinic operation through collaborative support and working relationships with a team approach with all CHWP staff. Serves as a mentor and coach to team members/staff resulting in a cohesive approach to providing services to CHWP’s patients. Compliant with HIPAA Primary Tasks and Duties Comprehensive physical assessment; evaluating, diagnosing, and treating new and existing patient's medical conditions. Initiating and interpreting labs and diagnostic studies. Performing medical procedures. Prescribing and referring patients for specialized consultation. Assisting Physicians in medical procedures. Using prescriptive authority to write prescription medicines for patients. Electronically record progress notes on patients' charts indicating patient status and treatment procedures performed. Conducting follow-up patient care. Providing health education to patients and families. Supervising and/or coordinating the activities of the patient care team. Teaching and training illness prevention. Actively participate in community health education. Performing emergency life-saving procedures in cases such as cardiac arrest, respiratory arrest, massive hemorrhage, or similar emergencies. Are among front-line medical providers in emergency disaster services. Experience/Knowledge of accreditation process and requirements, as well as all federal, state and local regulations and standards associated with the delivery of care in a community health center environment.     Ability to manage multiple responsibilities and emergency situations successfully Proficiency in knowledge of electronic medical record software Proficiency in knowledge of office management, use of computer, software packages, and office machines.
