Location: Burlington or Gloucester County, NJ
Job Type: Full-time, Part-time, Per Diem
Salary: $61-$64 per point + mileage reimbursement, benefits, 401k, paid time off
About the Role
Join our compassionate home health team as a Physical Therapist, helping patients regain mobility and independence in the comfort of their homes.
Key Responsibilities
- Conduct physical therapy evaluations and OASIS assessments
- Develop and implement individualized treatment plans
- Provide therapeutic interventions to improve patient mobility and function
- Educate patients and caregivers on home exercise programs
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Maintain accurate and timely documentation
Qualifications
- Current Physical Therapist license in New Jersey
- Master's or Doctorate in Physical Therapy
- Minimum of 6 months of hands-on therapy experience
- Valid driver's license, reliable transportation, and car insurance
- CPR certification for healthcare providers
