Job Summary:

We are seeking a compassionate and dedicated Social Service Director to join our team to provide services to our short term and long term guests.  As a Social Worker, you will be responsible for assessing the psychosocial needs of our guests and developing care plans that promote their overall well-being. You will work closely with our interdisciplinary team to ensure that our guests receive the highest quality of care.


Essential Functions:

  • The social worker will work to appropriately transition the resident and family through the care process
  • Implement healthcare directives and ability to have discussions and knowledge of end of life care and hospice/palliative care.
  • Understand fundamentals of discharge planning and able to work collaboratively with healthcare team which includes nursing and physician
  • Conduct psychosocial and cognitive assessments of guests to identify their needs and preferences
  • Develop care plans that address the social, emotional, and psychological needs of guests
  • Collaborate with the interdisciplinary team to ensure that care plans are implemented effectively
  • Provide counseling and support to guests and their families
  • Advocate for guests and ensure that their rights are protected
  • Maintain accurate and up-to-date guest records
  • Participate in team meetings and care conferences
  • Provide education and training to staff and families on social work issues

Qualifications/Experience:

  • Minimum of 2-3 years’ experience in long term care and short term rehab
  • Must hold a Bachelors degree
  • Must demonstrate strong conflict resolution
  • Excellent communication and interpersonal skills
  • Knowledge of federal and state regulations related to long term care
  • Strong organizational and time management skills
  • Compassionate and empathetic demeanor
  • Knowledge of Point Click Care is preferred
