Compensation: $39-$60.52 per hour, based on experience.
Status: Casual Status-23 or less hours per week.
Hours: 8:00 AM to 5:00 PM, Monday-Friday. Work outside of these hours/days may be required. Scheduled to be determined by the supervisor.
Location: This role involves local/regional travel throughout Central Oregon via personal vehicle during the workday, including visits to patients' homes and facilities.

Summary/Objective: The Physical Therapist plans, coordinates, implements and evaluates home health and hospice therapy services. The Physical Therapist is knowledgeable in providing therapy in the home setting. The therapist builds from the resources of the organization and community to provide services to meet the needs of individuals and families within their homes. The Physical Therapist is responsible for cost effective and effective patient care with quality outcomes.

Essential Functions:

Complete an initial comprehensive (if HH includes OASIS) and accurate assessment of patient and family to determine home health needs.
Develop an individualized care plan, which establishes measurable goals based on the therapy assessment including the patient’s functional abilities and limitations, rehab potential, and the patient’s diagnosis.
Collaborate with the patient/family and physician in the planning process.
Make necessary revisions to the care plan as patient status and needs change and update the physician as appropriate.
Identify patient family needs for other home health services as necessary.
Initiate appropriate preventative and rehabilitation treatment and procedures.
Provide physical therapy instructions and HEP education to patient/family/caregivers as appropriate. Adapt teaching styles to meet patient and family needs.
Actively promote and participate in Fall Intervention Program for patients.
Ensure that arrangements for equipment and other necessary items and services are available and provided.
Identify discharge-planning needs as part of the care plan development and implement prior to discharge of patient.
Delegate appropriate plan of care to licensed physical therapist assistant. Supervise licensed physical therapist assistant at least every 60 days and more frequently as stability of patient condition/progress warrants.
Develop, monitor and update the Home Health Aide Plan of care as necessary.
Instruct, supervise, and evaluate home health aide care provided every 14 days.
Communicate and update the patients plan of care if needed when patient status changes.
Communicate with the physician regarding the patient’s needs, and report changes in the patient’s condition; obtain/receive physician’s orders as required.
Communicate and coordinate with all members of the healthcare team to coordinate the care plan including community resources.
Attend staff meetings, case conferences and IDT as required.

Work Environment:

May be exposed to infections and contagious diseases. Contact with patients under wide variety of circumstances.
May be exposed to/occasionally exposed to patient elements. Subject to varying and unpredictable situations.
Handles emergency or crisis situations.
OSHA exposure category 1: Position includes tasks that involve exposure to blood, body fluids, tissues.
Required Personal Protective Equipment as conditions warrant:Gloves, gown, mask, mask with shield, goggles, shoe covers, bouffant cap.

Required Skills & Qualifications:

A degree from a baccalaureate or master’s program in physical therapy approved by an accredited organization.
Licensed to practice as physical therapist within the state.
Two years of appropriate experience as a physical therapist. Community/home health experience is preferred.
Ability to pass a background test, drug screen, and TB test.
Valid CPR Certification.
Current automobile insurance, valid Oregon driver’s license, and reliable transportation.
Strong communication and interpersonal skills.
Ability to work independently and collaboratively within an interdisciplinary team.
