Don't miss an opportunity to join a fantastic team and a growing organization!   Position Overview:   The Behavioral Health Home Organization Nurse Care Manager (BHHO NCM) carries out medical functions to all clients receiving services from the BHHO team. Medical functions include basic psychiatric health assessments, treatment, monitor medications, education, and coordination of health care provided to clients in the community. The BHHO NCM will also be responsible for assuming lead role in developing and directing rehabilitation services to a designated caseload of clients.
