Role Summary
The LNA supports safe, efficient, patient-centered care in an outpatient Federally Qualified Health Center (FQHC). This role assists the care team with patient flow, rooming, basic clinical support, documentation, infection prevention, and maintaining an organized clinical environment. All duties are performed under appropriate clinical supervision and within New Hampshire LNA scope, organizational policy, and demonstrated competency.
Key Responsibilities
• Prepare exam rooms and support daily clinic flow
• Escort, room, and assist patients during visits
• Obtain and document basic patient data such as height, weight, temperature, pulse, respirations, blood pressure, oxygen saturation, and pain score
• Observe patients and promptly report concerns or changes to clinical support staff
• Assist providers and nurses during exams and approved procedures
• Support infection prevention, room turnover, equipment cleaning, and supply stocking
• Document assigned information accurately in the electronic health record
• Reinforce instructions already provided by clinical support staff
• Support a respectful, culturally responsive, team-based care environment
• Assist with quality improvement and patient access goals
Qualifications
• Current active New Hampshire LNA license
• CPR/BLS certification or ability to obtain within required timeframe
• Strong communication, organization, and teamwork skills
• Basic computer and electronic health record skills
• Ability to maintain confidentiality and professionalism
• Prior ambulatory, primary care, or community health experience preferred
Important Role Notes
• This role practices under the supervision of an RN, APRN, or LPN
• The LNA does not independently assess, triage, diagnose, or provide clinical advice
• All duties must remain within legal scope, training, and validated competency
• Additional tasks may be assigned as needed.
