Responsibilities

Hanover Hill Behavioral Health, located in Bethlehem, Pennsylvania, combines two leaders in health care, in the partnership of Universal Health Services & Lehigh Valley Health Network. This brand new 144-bed free standing behavioral health facility offers the community high-quality behavioral health care services for seniors, adults and adolescents.


Hanover Hill is seeking an experienced Care Navigator. This is a fantastic opportunity to join a multi-disciplinary team while working in a state-of-the-art facility, including EPIC. 

 

The Care Navigator is responsible for meeting with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The Care Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes

 

Schedule: Monday - Friday between 8:00 am - 4:30 pm

Qualifications

  • Bachelor’s degree is required.
