Provides care to optimize functional recovery of patient in collaboration with Physical Therapist
• Collaborates with physical therapists and health care team to implement care plan
• Provides patient care interventions based on patient acuity and functional status
• Follows the plan of care and reports on patient's status and progress towards goals
• Participates in planning and operational changes to achieve team and individual performance improvement goals
• Observes and documents patient's response to treatment and progress
• Assures accurate and efficient documentation of patient progress in patient record
• In collaboration with Physical Therapist communicates patient care needs with interdisciplinary health care teams
• Facilitates patient education and functional maintenance with patients/ families following the plan of care
• Serves as a patient advocate in identifying functional potential and promoting the appropriate utilization of services
Required Education: Associate's Degree of Associate of Applied Science and successful completion of an accredited Physical Therapist Assistant (PTA) program
Required License: Valid MS State Board of Physical Therapy (MSBPT) Physical Therapist Assistant (PTA) license; or temporary 90-day licensure issued by the MSBPT; Temporary license must be obtained and submitted prior to hire date
