Job DetailsJob Location: Binghamton, NY 13901Position Type: Full-TimeSalary Range: $59,500.00 - $59,500.00 SalaryJob Shift: DayThe Licensed Practical Nurse is responsible for overseeing the medical needs of youth within Foster Care including conducting screenings, treatment planning, case management, care coordination, health promotion and youth and family support and services while using technology to link services. Description Job Responsibilities Conduct screenings, including HIV risk screenings, intake screenings, and obtain family health history. Routine screening for substance abuse, developmental health, mental health, and coordinate physical body checks for abuse. Routine management and training regarding health education. Establishing treatment goals, utilizing historical & current available information in collaboration with multidisciplinary team. Reinforce instruction of medication for youth and both birth and foster families on medication administration. Assist Psychiatric Mental Health Nurse Practitioner: Complete Prior Authorizations Call pharmacies for medication verification Call/ fax labs to schedule lab draws and get results Call providers for follow up and records release Educate families and youth of safe medication storage and possible side effects to watch for. Medication reconciliation and monitoring of medication. Support youth and families on how to maintain medical supplies and equipment. Follow up after medical/dental appointments, community provider visits, urgent/emergency care, or hospitalization. Medical Case Management and documentation for medical, dental and behavioral health appointments and updates. Maintains compliance with state and agency law and regulations. Adheres to agency policies and procedures; follows and supports agency mission, vision and values including participating in internal surveys and data collection for continuous improvement initiatives. Practicing proper infection control. Performs other duties as assigned   Job Requirements Current New York State Licensed Practical Nurse license and CPR/First Aid certification. Minimum 1 – 3 years of experience. Experience with at-risk youth preferred. Flexible schedule and ability to provide weekend, holiday, and On-Call coverage. Must have current, valid driver’s license per Agency guidelines. Travel required.
