What You’ll Do

As a Patient Resource Coordinator, you’ll help patients overcome social, financial, and environmental barriers so they can access and continue care. You’ll coordinate enabling services, connect individuals and families to internal and community supports, and provide limited clinical assistance within the CNA scope to support efficient visits.

Key responsibilities include:
- Linking patients to community resources, financial assistance, and insurance coverage options
- Coordinating with medical, dental, behavioral health, care management, and patient services teams to improve continuity of care
- Supporting patient navigation and reducing obstacles to treatment
- Assisting with patient flow and basic clinical tasks within CNA scope

Required qualifications:
- Current North Dakota CNA certification
- Certified Application Counselor (CAC) certification preferred or ability to obtain within 90 days
- Willingness to travel between locations; occasional evening/weekend availability as needed

A Typical Shift

Your day blends patient conversations with coordination work: you may start by checking in with care teams on patients who need extra support, then spend time on calls or referrals with community partners and assistance programs. Between patient touchpoints, you’ll document progress, track follow-ups, and help keep visits moving smoothly. Some days include outreach or travel between Northland Health Centers locations, with occasional evening or weekend activities when community needs require it.
