Case Manager - Medical Services

New River Valley Community Services is seeking a Case Manager for our Medical Services Team! The Case Manager provides needs assessments, links resources, and monitors services for individuals with substance use and/or co-occurring disorders with the MAT (Medication Assisted Treatment) Team. This role coordinates services and resources for clients as needed to maintain in their communities; in addition to monitoring services as they are delivered to ensure consumers' needs are met. The Case Manager also ensures documentation process requirements are met accordingly and also assists with transportation and community integration as needed.

Starting Hourly Rate: $21.56-Depending on Experience

Education Requirements: 

• Bachelor's Degree in Education, Social Work, Psychology or Human Services related field is required with at least one year experience in a setting working with individuals with disabilities, mental health needs, substance use or Developmental Disabilities is preferred.
• QMHP is required through the Virginia Board of Counseling.
• CSAC is beneficial.
• Previous Case Management experience is preferred.

Additional compensation will be considered for any new employees who speak multiple languages.
