Client Support Aide

 

General Description:

Assists with daily living and working activities in a training or residential facility.  Observes and monitors

client’s behavior and charts observations and incidents.  Provides training of basic personal and social

skills, simple academics, and work skills to Clients in a workshop environment or residential group

home. Encourage client group activity and provide limited case management.

 

Position Qualifications:

6 months experience providing social or direct care services to groups of special needs individuals in a  training or residential environment.

 

Knowledge, Skill, & Ability:

Knowledge of pertinent state, federal and local laws, rules and regulations.

Ability to identify and report facility repairs and equipment problems.

Ability to serve as a liaison between all interested parties.

Ability to serve as a member of the treatment team to develop participants’ goals and objectives.

Ability to complete participant orientation sessions.

Ability to train participants in personal hygiene and other daily living activities.

Working Conditions: 

Employee is frequently required to walk, sit, stand, bend, squat or kneel and occasionally required to climb or 

balance and stoop. Employee must occasionally lift and/or move 50-75 pounds.  With proper training, employee

Is responsible for transferring and positioning client.  Must have ability to sit for long periods of time at a computer. 

Lighting and temperature are adequate, and there are no 

hazardous or unpleasant conditions caused by noise, dust, etc. Employee must be able to travel

Reporting to work when scheduled;

Begins and ends work as expected.

Reporting to supervisor upon knowledge of tardiness or absence.

Acts to maximize revenue and minimize expenditures consistent with providing quality services compliant with all applicable regulatory requirements.


Performance Goals:

Will begin and end the work day when scheduled and adhere to the rules, regulations, policies and guidelines

Employee will request and use PTO leave in accordance to policy

Staff will dress appropriately and according to Unison’s policy and site dress code

Health, safety and sanitation policies will be observed

Will not report to work under the influence of alcohol or drugs or engage in activities other than official business during working hours.

Performance Objectives & Goals:

 

Client Care

It is the responsibility of all staff to insure that the persons served through our programs receives services that address their individual needs.

All staff will uphold and demonstrate the values and mission of Unison BH.

Meets applicable productivity requirements in terms of direct service time – document services the same day they are provided. – Exceptions made for events operating beyond regular working hours. 

Instructs and advises clients on daily living and work skills in one-on-one or group instruction.

Observes and monitors client behavior. Provides vital support to client.  Transports, baths and feeds clients. 

Maintain order in home or program.  Maintains clean environment in home or program. 

Counsels and advises clients on behavioral problems, daily decision-making, and the resolution of minor problems. 

Assists with the development of an individual service/treatment plan for each assigned Client by completing Person Centered documents prior to ISP meeting.

Participates in case reviews, consulting with the team to evaluate the client's needs and plan for continuing services. Counsels and advises clients on behavioral problems, daily decision-making, and the resolution of minor problems.                    

               

Performance Goals:

Instructs, assists and supervises clients in one on one or group instruction

Follow ISP goals and daily lessons while providing services

Attend ISP meetings and report any changes/needs/client preferences to program Case Manager. 

                    

Transporting

Operates vehicle to transport clients on routes, to program and home each day.

Assist Clients on/off vans daily.

Transports clients to community access activities

Provide transportation to/from center and outings.

Ensure all passengers follow rules according to DMV.

Follow all laws and guidelines while driving and transporting Clients.

 

Performance Goals:

Ensure weekly inspections are completed on assigned vehicle and sent to supervisor by the following Monday.

Aid individuals entering/exiting van

Ensure van is clean and maintains at least a ½ tank of gas after each trip. 

 

CAG Development and Training

Responsible for instruction and training of CAG activities.

Responsible for engaging Clients in planning community activities of their interest. 

Responsible for engaging Clients in participating in community activities of their choice.

Responsible for instruction on assigned weekly topics and obtaining materials covering topics. 

 

Performance Goals:

Ensure Clients are taught according to ISP goals 100%.    

Develop weekly lessons and activities with client input.  To be submitted prior to new monthly calendar.

Responsible for weekly client community involvement including but not limited to; utilizing public resources, volunteering, shopping, clubs, events, etc.… for increased social interaction and development.

 

Record Management

Prepares and maintains records of client progress and services performed, reporting changes in client condition to manager or supervisor.

Ensure all documentation assigned follows agency policy and Medicaid requirements.

Ensure all reviews and annual paperwork is completed prior to review date.

Required to document services the same day services are rendered. Upon request Instructor should send documentation to supervisor in the form for and alert for them to review and release to billing. All documentation of services will address ISP goals and will correspond with amount and frequency outlined in ISP.


Performance Goals:

Maintain a minimum score of 85% on assigned section of Audits. 

Ensure all documentation is accurate according to the client’s current goals and objectives.

Document on services provided by the end of the day for each client and provides enough information to support the billing of services.

Documentation will clearly reflect skill building that is appropriate for the services being billed.

Report any billing/documentation errors by submitting a Help Desk request as soon as error is discovered.
