Sage Group HealthCare Staffing is seeking a Registered Nurse Home Health for a 13-week travel/contract position in Bloomington, IN Accepting both local and traveler candidates.

Pay Rate: Range $2160 to $2200 Weekly Gross pay based on a 40-hour work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments

Unit: Home Health

Shift: 8 Hours; Days, 8.00a - 5.00p

Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri, Productivity expectation - 32 points per week.

Position Requirements:

Registered Nurse: 2 years - Required
Recent/Current Home Health Nursing Experience: 1 year within the past 3 years – Required
OASIS experience: Recent/current - Required
Home Care Home Base (HCHB) charting experience: Recent/current - Required
Must be Covid Vaccinated - Declination/s accepted for this position, Medicial or Religious exemptions (if qualified) may be considered on case to case basis

Education, License and Certification Requirements:

ASN or an equivalent – Required
BSN or an equivalent – Highly preferred
RN License for State applying or compact if accepted in the state - Required
BLS Certification from AHA - Required

Job Description:

Provide and coordinate care for assigned Home Health patients
Develop and implement nursing care plans for patients as assigned.
Empower patients and families to reach healthcare goals by educating them on disease management.
Provide Home Health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
Notify physicians of significant changes in status.
Document and complete charting
