Work Schedule:

  • Full-Time: Monday-Friday

  • Day Shift: (9am-5:30pm)

Office Coordinator II Job Overview

This position is a multi-functional position that serves as an organizational and problem-solving contact in the office setting. Under minimal supervision, supports staff, management, and/or medical personnel with executing specific duties essential to the service or business being provided by the department/office. These duties may include data entry, billing, recordkeeping, correspondence, patient contact, registration, time/attendance, scheduling of staff, contractors, patients, or customers, and monitoring/upkeep of equipment, technology, and/or supplies. May conduct and oversee clerical duties including answering of telephones, routing of incoming calls, problem resolution, and customer service. May provide work direction and mentorship to other clerical staff. Other duties of a clerical nature essential to the specific business unit may also be included as assigned.

This position will primarily support the timely scheduling of all incoming PalliaCare referrals with the operational expectation of achieving 80% or greater of new referrals scheduled within seven (7) calendar days, and our strategic goal to serve patients discharged from a hospital within 2 days and grow our census in Assisted Living and Long-Term Care, aligning with our strategic goal.  The role will also provide critical support for referral processing, provider scheduling, communication management, Epic documentation, and coordination across the interdisciplinary team. 
 

The PalliaCare Office Coordinator serves as the operational hub for the outpatient palliative care program by coordinating referrals, scheduling new consultations, managing patient communications, processing documentation, and supporting providers and clinical staff. Responsibilities include:

  • Processing internal and external referrals
  • Managing Epic referral work queues
  • Scheduling new patient consultations and follow-up appointments
  • Coordinating provider schedules to maximize efficiency
  • Completing patient registration and demographic verification
  • Managing patient outreach and appointment reminders
  • Processing medical records and referral documentation
    Job Requirements
  • Monitoring multiple communication platforms, including Epic, MyChart, department email, Voalte, and telephone communications. 
    High School Degree
    Computer proficiency in MS Office 
    Medical terminology 
    Verbal & written communication 
    Customer service 
    Negotiation skills 
    Independent thinker
    3-4 years' experience Clerical in a related field
