Overview
We are seeking an Inpatient Pharmacist, also referred to as an Acute Care Pharmacist or Hospital Pharmacist. As a Hospital Pharmacist for our Inpatient Pharmacy, you will prepare, dispense, and manage medications for patients in our hospital under the supervision of our Pharmacy Director. Hospital Pharmacists work closely with doctors and nurses to ensure medications are safe, effective, and correctly dosed, while also monitoring for side effects or drug interactions.Â

Responsibilities
Essential Duties and Responsibilities (includes, but not limited to):Â
- Conduct patient interviews to obtain accurate and complete medication histories during admission or transition of careÂ
- Review and verify physician medication orders for appropriateness, dosage, route of administration, frequency, and potential drug interactionsÂ
- Ensure medication orders comply with hospital protocols, clinical guidelines, and patient-specific factors (e.g., renal function, allergies, weight)Â
- Prepare and dispense medications accurately, including IV admixtures, unit-dose medications, and controlled substances, following all safety and sterility protocolsÂ
- Monitor medication therapies and provide clinical recommendations to optimize therapeutic outcomes and minimize adverse drug eventsÂ
- Collaborate with physicians, nurses, and other healthcare professionals to develop and adjust patient-specific medication plansÂ
- Coordinate daily operations within the pharmacy to ensure efficient medication distribution and inventory managementÂ
- Deliver pharmaceutical care to a wide range of inpatient units, including – General Medicine/Surgery, Intensive Care Units (ICU), Obstetrics & Gynecology, Pediatrics, Emergency Department, Oncology, Perioperative Services, Geriatrics and other departmentsÂ
Qualifications
EDUCATION, EXPERIENCE, TRAINING

1. Current state Registered Pharmacist License.2. Experience in hospital pharmacy operations preferred.3. B.S. degree in Pharmacy required; Doctor of Pharmacy degree preferred. 4. BLS and ACLS certification preferred
