Speech-Language Pathologist (SLP) | Full-time | Outpatient | 2131 College Ave Bluefield, VA 24605

H2 Health is seeking a Speech-Language Pathologist to join our outpatient team in Bluefield, Virginia. If you're passionate about helping patients strengthen communication, improve cognitive function, and enhance their overall quality of life, you'll find a rewarding opportunity in a collaborative environment that values clinical excellence and professional growth. Whether you're an experienced SLP or a Clinical Fellow beginning your career, you'll be supported by a team committed to exceptional patient care and ongoing development.

In this role, you'll work with a diverse outpatient population while providing individualized, evidence-based therapy services. You'll collaborate with Physical Therapists, Occupational Therapists, physicians, caregivers, and other healthcare professionals to help patients maximize communication, cognition, swallowing function, and independence.

Your Role

  • Perform comprehensive speech-language evaluations and develop individualized plans of care.
  • Assess and treat communication, language, cognitive-linguistic, voice, fluency, and swallowing disorders.
  • Deliver evidence-based interventions designed to improve communication skills, cognition, safety, and quality of life.
  • Collaborate with patients, caregivers, physicians, and interdisciplinary team members to achieve optimal outcomes.
  • Educate patients and families on treatment strategies and home programs.
  • Utilize Scribe AI-powered documentation technology to streamline documentation and maximize patient care time.
  • Utilize the Raintree EMR platform for documentation, scheduling, and patient management.
  • Master's degree in Speech-Language Pathology from an accredited program.
  • Current Virginia Speech-Language Pathologist license or eligibility for licensure.
  • Certificate of Clinical Competence (CCC-SLP) preferred or eligibility to complete Clinical Fellowship Year (CFY).
  • CPR certification or willingness to obtain as required.
  • Strong communication and interpersonal skills.
  • Commitment to providing high-quality, patient-centered care.
