Primary Care Provider — NP/PA (Autonomous Clinical Role)

SUNDARA Medical is seeking a highly experienced Primary Care Provider (NP or PA) who is ready to step into a fully autonomous, high-responsibility clinical role within a fast-growing, non-traditional healthcare organization.

This is not a training role. This is not a supervised or heavily structured environment.

We are building a different kind of healthcare company—one that believes primary care should be personal, proactive, accessible, and centered around the patient rather than constrained by legacy systems.

We are a scrappy, high-growth startup. That means we move quickly, solve problems in real time, and expect every team member to operate with ownership, initiative, and clinical confidence.

If you need extensive structure, step-by-step protocols for every scenario, or close oversight to practice safely, this is not the right fit.

The Role

As a Primary Care Provider at SUNDARA Medical, you will independently manage a full-spectrum primary care panel with a strong emphasis on prevention, chronic disease management, acute care, and long-term patient relationships.

You will not be micromanaged. You will be expected to function as a self-directed clinical decision-maker within your scope of practice.

This role is both clinical and foundational to the organization. You will help shape how care is delivered, how workflows are built, and how patients experience healthcare at SUNDARA.

We are actively building the system while operating inside it.

What You Will Do

  • Independently provide comprehensive primary and preventive care
  • Diagnose and manage acute and chronic conditions without routine oversight
  • Perform annual wellness and preventive visits
  • Order and interpret labs, imaging, and diagnostic testing
  • Develop and manage individualized treatment plans
  • Coordinate specialty referrals as clinically appropriate
  • Maintain accurate, timely, and compliant documentation
  • Build and maintain long-term patient relationships
  • Operate as a fully accountable provider for your patient panel
  • Collaborate with leadership and clinical peers as needed—not for approval, but for alignment
  • Identify inefficiencies in care delivery and actively help improve them
  • Contribute to the development of clinical workflows, protocols, and systems
  • Participate in building new programs and service lines as the organization grows


Compensation (RVU-Based — Collected Revenue Only)

This position is compensated through a direct percentage of actual dollars collected, not billed charges or theoretical RVUs.

We believe providers should be aligned with real revenue and real outcomes.

Monthly Collection-Based Structure:

  • $0 – $19,999 collected/month → 35% of collected revenue paid to provider
  • $20,000 – $29,999 collected/month → 40% of collected revenue paid to provider
  • $30,000+ collected/month → 45% of collected revenue paid to provider

Important Notes:

  • Compensation is based strictly on actual dollars collected
  • No credit is given for uncollected or denied claims
  • This structure is designed to reward efficiency, clinical productivity, and strong patient panel development
  • High-performing providers have meaningful upside potential


Credentialing Requirement (Non-Negotiable)

Because this role is designed for immediate clinical productivity, prior insurance credentialing in Utah is required.

You must already be credentialed (or have recent, verifiable prior credentialing) with major commercial and/or government payers in Utah.

We are not able to support initial credentialing for this role.

Please include your full payer credentialing history in your application.


Clinical Independence Requirement

This role requires:

  • Demonstrated ability to practice independently and safely within scope
  • Strong prior experience in primary care, family medicine, or internal medicine
  • Comfort managing a full patient panel without routine supervisory review
  • Clinical maturity to escalate only when appropriate—not by default

We are specifically seeking providers who are already operating at a high level of autonomy in outpatient care.


What We’re Looking For

Required:

  • Licensed NP or PA eligible to practice in Utah
  • Significant prior primary care experience
  • Ability to practice independently with minimal oversight
  • Active or prior Utah payer credentialing (required)
  • Strong clinical judgment and decision-making ability
  • Excellent communication and patient relationship skills
  • High level of organization and documentation discipline

  • You will be expected to help define solutions, not wait for them

If you prefer highly structured environments with rigid protocols and extensive oversight, this will not be a good fit.

If you thrive in environments where you can build, iterate, and lead from the clinical front lines, you will likely excel here.
