As we expand into Bluffton, South Carolina, we are seeking an experienced Family Nurse Practitioner or Physician Assistant who shares this commitment to high-touch, patient-focused care.

Position Summary:
This role is ideal for a clinician who wants to step away from high-volume, transactional healthcare and practice in a setting that values connection, time, proactive wellness, and individualized attention. You will manage a smaller membership panel and provide comprehensive, relationship-based care.

Primary office hours: Monday–Friday, 8:30 AM – 5:00 PM

Additional expectations: Must be willing to take concierge member texts/calls after hours and on weekends as part of the membership experience.

Key Responsibilities:
• Deliver exceptional, patient-centered care to concierge members.
• Provide wellness visits, acute care, chronic disease management, and individualized care plans.
• Maintain a solution-focused mindset and actively work to “make yes happen” for members.
• Offer same-day or next-day access as appropriate for concierge patients.
• Communicate promptly and compassionately with members via phone, text, secure messaging, and in-office visits.
• Collaborate with a supportive administrative and clinical team to ensure seamless care coordination.
• Represent Coastal Care Partners with professionalism, warmth, and a commitment to excellence.

Requirements

-Licensed Family Nurse Practitioner or Physician Assistant in South Carolina.
-Minimum 3 years of experience in family medicine, primary care, internal medicine, or concierge medicine.
-Strong interpersonal communication skills and dedication to long-term patient relationships.
-Ability to practice independently while working collaboratively within a team.
-Experience with AthenaHealth EMR preferred.
