Job DetailsJob Location: Family Support (13) - Needham, MA 02492Position Type: Full TimeSalary Range: $25.00 - $27.00 HourlyJob Shift: AnyPosition Overview The Family Support Center at Charles River Center is seeking a Family Transition & Community Resource Coordinator to support individuals with developmental disabilities and their families as they navigate the transition to adulthood and adult services. This role will work collaboratively with the local Department of Developmental Services (DDS) Area Office and the Charles River Center Family Support Center to help families access critical resources, plan for adult services, and build meaningful community connections. This position will be based at the DDS Area Office approximately three days per week and at the Charles River Center Family Support Center the remaining days. Key Responsibilities Family Transition Support Provide guidance to families/individuals as they approach age 18 and transition to adult services. Assist families in accessing public benefits and resources including MassHealth, Supplemental Security Income (SSI), and other generic supports. Provide information on guardianship and alternatives to guardianship and support families in understanding the adult service system. Post-22 Planning Help families identify post-22 day programs and community-based service options. Assist with referrals, program exploration, and coordinating tours of potential day programs. Provide information on employment and community-based adult services. Community Connections Collaborate with the DDS Self-Direction team to support individuals in building meaningful community involvement. Identify and help develop partnerships with community organizations such as museums, YMCA programs, art and music programs, and other recreational opportunities. Support individuals and families in accessing inclusive community activities. Collaboration & Outreach Work closely with DDS Service Coordinators and Family Support Center staff to identify gaps in community access and solutions. Participate in family workshops, outreach events, and resource sharing. Maintain knowledge of local services and resources available to individuals and families. QualificationsQualifications Bachelor’s degree in Human Services, Social Work, Special Education, Psychology, or a related field, or equivalent experience. Experience working with individuals with developmental disabilities and their families. Strong communication, organizational, and collaboration skills. Ability to work both independently and as part of a team. Preferred Knowledge of MassHealth, SSI, and Massachusetts adult service systems. Experience with transition planning for youth with disabilities (18–22). Familiarity with DDS services, self-direction, and adult day program options.
