The purpose of the Clinical Case Manager is to work collaboratively with the health care provider(s), interdisciplinary team, patients and community health partners to coordinate care for individuals with chronic or complex conditions, or those requiring high levels of care coordination. The Clinical Case Manager is responsible for assisting with Transitions of Care post hospitalization.
