First Choice Hospice is looking for full time Hospice On-Call Registered Nurses (RN) who are interested in caring for our community-based patients enrolled in our hospice program in the Boise, ID and surrounding areas.At Jet Health, Inc we strive to offer a true sense of belonging for all our employees. Requirements
- Follow a designated care plan in accordance with patient's needs
- Make home visits to clients in designated geographic territories
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures
- Monitor clients' conditions, reporting changes to Clinical or Client Services Manager
- Follow up with, execute, and properly document doctors' orders
- Perform client assessments
- Case management and coordination
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
Qualifications:
- Current State or Compact Registered Nurse (RN) License in hand
- A minimum of one-year clinical experience is preferred
- Demonstrate knowledge and skillset in current nursing practice
- Possess a valid state driver's license and automobile liability insurance
- HCHB (Home Care Home Base) EMR knowledge is desired
