To cultivate spiritual and emotional support to patients, 聽families/caregivers to ensure each have a voice to explore spiritual 聽concerns; ask crucial questions; comprehend the importance of their 聽lives; and discover peace as they approach death.聽
Requirements
ESSENTIAL DUTIES & RESPONSIBILITIES:聽
路 Assess the spiritual needs of patient/family with patient/family consent聽
路 Provide spiritual care and support for patient/family/caregivers as needed or requested聽
路 聽Develop a plan of care with the interdisciplinary team meeting the 聽spiritual/emotional needs of the 聽patient/family caregivers and update 聽the plan of care every 14 days聽
路 Utilize spiritual resources to meet needs of patient and family聽
路 Consult and provide education to interdisciplinary team about spirituality and spiritual care聽
路 Provide ethical consultation for staff regarding end-of-life issues and how faith impacts choices聽
路 Contact patient/family鈥檚 minister and other religious caregivers, as appropriate and as requested by 聽patient/family聽
路 Serve palliative care patients as assigned聽
路 Be available to attend receivings or funerals for patients on weekdays聽
路 Facilitate or assist with funerals/memorial services, as requested聽
路 聽Plan and lead occasional worship services for patients/family members, 聽staff, and facilities in collaboration 聽with Director of Faith Outreach聽
路 Coordinate memorial services with long-term care staff, as directed by facility聽
路 Provide spiritual care and support for bereaved family members upon request聽
路 Maintain Hospice bereavement family caseload as requested聽
路 Complete initial bereavement assignment for patient families served as requested聽
路 Develop appropriate bereavement plan of care and update quarterly as requested聽
路 聽Participate in the planning and facilitating of grief support 聽groups/presentation, as needed, in the community 聽in collaboration with 聽Grief Support Specialist聽
路 Assist with the annual memorial service with Grief Services Director聽
路 Facilitate (if trained) Walking the Mourner鈥檚 Path program and Limit of Our Sight 聽
路 Rotate chaplain on call responsibilities, minimum 2x a year or as requested/needed聽
路 Collaborate with on-call weekend social worker and nurse regarding patient family needs聽
路 Attend funeral/receiving scheduled on weekends when on-call聽
路 Serve as a resource to community clergy and religious caregivers about Hospice, death, dying, and grief 聽issues聽
路 聽Provide education and consultation to community clergy and groups to 聽enhance their sensitivity to the 聽spiritual concerns of patient and 聽caregivers experiencing illness and loss聽
路 Support community clergy/congregations through the process of caring for the hospice patient/family聽
路 Participate in volunteer/staff orientation/training, as requested聽
路 Assist with training volunteer chaplains, as needed聽
路 Complete all Relias trainings and attend additional mandatory trainings聽
路 Other duties as assigned聽
KNOWLEDGE, SKILLS & ABILITIES: 聽
路 聽Ability to identify and respect the spiritual needs of the 聽patient/families/caregivers as they differ according to 聽social status, 聽religious beliefs, sexual orientation, race, age, and their emotional, 聽physical, and intellectual 聽capabilities聽
MINIMUM QUALIFICATIONS: 聽
Education: 聽
路 聽Master of Divinity degree or equivalent graduate degree that includes 聽theological studies from an accredited seminary or divinity school聽
路 Minimum of two units of Clinical Pastoral Education (CPE) completed or equivalent clinical training and experience聽
路 Endorsed by and in good standing with the denomination of ordination聽
Experience: 聽
路聽Minimum two years of experience working with individuals, groups and 聽families in crisis, preferably dealing with issues involved in death, 聽dying, and grief聽
Licensures & Certifications: (if applicable): 聽
路聽Certified or progressing toward certification by a cognate group 聽recognized by the Coalition on Ministry in Specialized Setting (COMISS)聽
