Humana's Primary Care Organization is a fast‑growing provider of senior-focused, value‑based care, with 340+ centers across 15 states under the CenterWell and Conviva brands. We're transforming healthcare by putting seniors first. We're looking for a full-time Primary Care Physician to join our CenterWell team. This role is ideal for a compassionate, experienced clinician who excels in a collaborative, patient-centered setting focused on adult and geriatric care.

Main Responsibilities

  • You will deliver comprehensive outpatient care to adult and senior patients.
  • Maintain accurate and timely medical records and documentation.
  • Diagnose and manage moderately to complex medical conditions.
  • Coordinate referrals and collaborate with specialists.
  • Participate in clinical quality improvement projects.
  • Work with interdisciplinary teams to ensure holistic care.
  • Contribute to projects and innovations in care delivery.
  • Exercise independent clinical judgment in patient management.
  • You will report to the Physician Lead and Associate Medical Director.
  • This is a full-time onsite position.

Required Qualifications

  • MD or DO from an accredited medical school.
  • Active, unrestricted medical license in the state of practice.
  • Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
  • 2 years' experience in care or managing high-acuity geriatric patients.
  • Commitment to improving patient experience and outcomes.
  • Participation in Tuberculosis (TB) screening program.

Preferred Qualifications

  • Your specialty training in Family Medicine, Internal Medicine, Med-Peds, or Geriatrics.
  • Experience working with senior populations or in value-based care settings.
  • Proficiency with electronic health records (EHR) and digital documentation.
  • Strong communication, collaboration, and interpersonal skills.
  • Ability to work independently and adapt to evolving clinical environments.
  • Experience supervising Advanced Practice Providers (NPs/PAs).
  • Bilingual English/Spanish. Will speak, read and write in both languages without limitations or assistance. See Additional Information on testing

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
