Nurse Care Coordinator (RN/LPN) - HIV Waiver Program

Location: Augusta, Maine (Hybrid)
Pay Rate: $36.50 per hour
Schedule: Full-Time | Monday-Friday
Work Arrangement: Hybrid (2 Days In Office, 3 Days Remote Within Maine)
PTO: 15 Days Annually

Staffing Solutions Organization (SSO) is seeking a compassionate and experienced Nurse Care Coordinator (RN/LPN) to support the Maine Department of Health and Human Services' HIV Waiver Program.

In this rewarding role, you'll work directly with MaineCare members living with HIV/AIDS, helping them navigate healthcare systems, access essential services, and overcome barriers to care. You'll collaborate with healthcare providers, community organizations, and support networks to ensure members receive coordinated, person-centered care that improves health outcomes and quality of life.

Care Coordination & Member Support:

  • Provide clinical care management and ongoing support to MaineCare members living with HIV/AIDS.
  • Assess member needs and identify barriers to healthcare access.
  • Connect members with medical, behavioral health, social service, and community-based resources.
  • Promote continuity of care through effective coordination among providers and support organizations.

Collaboration & Partnership:

  • Work closely with physicians, nurses, case managers, community agencies, and other stakeholders.
  • Build and maintain positive working relationships with healthcare providers and community partners.
  • Support person-centered care planning and service delivery.

Program Compliance & Technical Assistance:

  • Apply federal and state regulations, MaineCare requirements, and program policies.
  • Provide technical guidance regarding program procedures and eligibility requirements.
  • Assist in resolving complex member care and service coordination issues.

Documentation & Quality Assurance:

  • Maintain accurate and timely documentation of member interactions and care coordination activities.
  • Monitor case progress and ensure compliance with program standards.
  • Participate in quality improvement efforts and corrective action initiatives as needed.

Professional Development & Program Participation:

  • Participate in trainings, conferences, committees, and workgroups.
  • Represent the program at HIV and Ryan White advisory meetings and other healthcare initiatives.
  • Support statewide efforts to improve care and services for individuals living with HIV/AIDS.

Required Qualifications:

  • Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Maine.
  • Minimum of three (3) years of progressively responsible experience in nursing and/or healthcare service delivery.
  • Strong knowledge of nursing practices, healthcare systems, and care coordination principles.
  • Excellent communication, documentation, and interpersonal skills.
  • Ability to work independently while managing multiple priorities.

Preferred Experience:

  • Care management or case management experience.
  • Experience working with HIV/AIDS populations, chronic disease management, or public health programs.
  • Knowledge of Medicaid/MaineCare programs and healthcare regulations.
  • Experience collaborating with community-based organizations and healthcare providers.

Compensation for roles at Staffing Solutions Organization LLC (SSO) varies depending on a wide array of factors including, but not limited to, the specific office location, role, skill set, and level of experience. As required by applicable law, SSO provides a reasonable range of compensation for this role. In addition, SSO provides a range of benefits for this role.

Applicants must be authorized to work in the U.S. This position is not eligible for work authorization sponsorship (including H-1B sponsorship) by SSO and under the federal laws relating to the F-1 STEM EAD program, this position is not eligible to support a STEM EAD extension application.
