General Responsibilities

  1. Conduct determination of eligibility assessments (DOEs) for older adults seeking waiver services while being enrolled with a Managed Care Organization. 
  2. Active intervention and advocacy to access necessary services from various programs and complete any necessary referral forms.  
  3. Complete proper termination of cases.
  4. Maintain confidential case records and meet documentation standards.
  5. Accurate completion of all required reporting and documentation.
  6. Maintain cooperative relationships with service providers and community organizations, including regular contact with Managed Care Organizations.  
  7. Submit accurate files to billing in a timely manner.  
  8. Attend training, staff, and team meetings, and act as an Agency ambassador in the community.
  9. Other duties as assigned within the guidelines of this position.

Requirements

  1. Bachelor's degree in social work, psychology, or related field from a regionally accredited institution. In some cases, a waiver may be granted by the Illinois Department on Aging if employee has significant case management or significant experience in aging services.  
  2. Minimum of three months of experience in a related field.
  3. Preferred experience with program-specific populations.
  4. Fundamental communication and problem-solving skills.
  5. Ability to work independently and respond appropriately in crises.
  6. Relate well to individuals with disabilities and older adults, respecting their privacy and dignity.
  7. Effective and constructive interaction with diverse cultures and backgrounds.
  8. Proficiency in Microsoft Office applications (Outlook, Word, Teams).
  9. Strong verbal and written communication skills.
  10. Adhere to the Agency’s Code of Ethics and conduct all responsibilities in accordance with the standards outlined in the Employee Handbook. 
  11. Valid driver's license, reliable transportation, and proof of liability insurance.
